We’re asking you to fill out a simple, 5-minute online assessment — just like a quiz. It’s a way of helping our health care team know about any chemicals — in cleaners, cosmetics, pesticides, or other products — that you might have contact with.

As you may know, chemicals can be harmful to your baby when you’re pregnant. The good news is that this assessment will give you tips on how to protect yourself and your baby. Your provider will automatically get your results too, so you can talk about it during your next appointment.

We also keep your personal medical information private — it’s protected under the federal Health Insurance Portability and Accountability Act (HIPAA).
